Brief moments where I run out of breath, have mild chest tightness, unpredictable for the most part; what is it?

I'm not sure what this is. I think it's anxiety, but I can't pinpoint exact causes.

I get random, short bursts where my chest tightens. I also feel like I'm about to vomit air (not in the sense of retching, but there's that feeling in my throat), and I have difficulty swallowing. None of these are symptoms that last persistently, but are kinda... episodic?

I take Nadolol for Long QT Syndrome. I'm asymptomatic, so it's more of a preventative for anything going on down the road. I've brought up one of these points with my cardiologist, but chalked it to anxiety. I wouldn't be surprised, with how easily overwhelmed I get.

I usually let it pass, and if I get the air stuck in throat feeling while talking, I take a moment to swallow before continuing.

Edit: this is all while I'm at rest, or at least not active to the point where it'd make sense for my heart rate to be up.

Edit: oddly specific, maybe, but I notice it more trying to fall asleep. Constant catches in throat (swallowing to alleviate it), weird nausea in head or stomach travelling up to head (it's the type of nausea where you know you're not gonna actually chuck, but it's there).
