Sitting on toilet a long time

My fiance bless his soul has been dealing with this issue and it seems to keep reoccurring.
The thing I was curious about is, does sitting on the toilet for a long time (30 minutes) cause the fistula to improperly heal? He sits on the toilet for a long time and this can't be healthy. Ive heard it can cause hemorrhoids but what about anything to do with fistula?
