How do I approach a friend about their alcoholism?

I have a friend who drinks a case of beer a day on average. He chain smokes as well and is just flat out looking rough.
I don't know how to break through to him and I know he has to want to quit first and I genuinely don't think he does at all, but I have to say something and at least try.
Any advice on how to approach him?
