CAC lockout fix?

Does anyone have insight as to why CACs are getting randomly locked out and credentials can not be verified? And a way to prevent it?
Mine got locked out yesterday. I went to MPF got it unlocked. Went back to my work center, tried to login and was immediately locked out again.
