How can I hold someone's attention/avoid someone interrupting me?

I'm 30 now and have always had an issue being interupted or not being paid attention to when speaking. This is especially bad at work as I'm in a management position.

The thing is, I'm not sure what I'm doing wrong. I'm good at public speaking, I'm not timid when I talk and have been told repeatedly that I have a loud and powerful voice.

It even happens at home, I'm speaking to my dad and my sister interrupts out of nowhere to ask a question to my dad. It's not even a fault of my sister or my parents as they are very kind people. But obviously I was doing something that implied she could freely inerupt.

Any advice? It gets extremely frustrating when I have to repeat instructions/things I'm saying in a group because I wasn't holding their attention or was interrupted.
