My wife thinks I don't care because I'm forgetful

I'm working on getting a formal ADHD diagnosis, but my wife agrees with my strong suspicion that I have it. In spite of this, she thinks my forgetfulness means I don't love her or care about our relationship, but it's the total opposite. I constantly tell her I wish she could see inside my head so she could see how I feel. Things will be great for a few weeks, and I'll really be on top of all the things she asks of me (helping with meal planning/cooking, doing laundry etc.), and then something comes along and all those things just drop off my radar and I'm back to my old ways and she feels like she's doing it alone. I hate the inconsistency and I don't know how to stop it. I'm currently in therapy about once a month and hoping that will help. I've massively reduced my alcohol consumption as well in the hopes it'll help me stay focused. Does anyone have any tips or tricks on remembering these daily tasks that help keep a household running? Any ideas on staying consistent?
