Difficult to focus on one thing

I am a graduate of computer science. A program that is normally supposed to be completed in 3 years took me four because when I first started, I had a hard time focusing on what the teachers said in class and also working on projects at home or doing any type of studying. My mind will always drift away to other things. Sometimes I can be hyper focused on something I am working on but it never lasts long and after that the avoiding kicks in and I keep telling myself I know what I have to do next and I know how to do it but I never actually do it. Personal projects are started but never completed. I always need some type of distraction to function.
