Really really really struggling to build routine

For the last 10 years I have been trying to build a routine and better habits. I get in the mood to do so I get excited and make plans it works for only a week or so before it all falls apart. Idk if I am trying to do too much at once or what. My wife told me to pick one or two things to work on and go from there, so I have picked oral health and doing the dishes/dinner clean up every night. Want to take better care of my teeth and set a good example for my son and tired of dishes piling up.
