Adderall suddenly stopped working yesterday after it worked for years

I was diagnosed with ADHD when I was 15. Took 40mg of Adderall XR daily in high school then dropped it to 20mg XR daily in college. Never had an issue with it, ever. After college I stopped taking it because I thought I wouldn’t need it as much now that I was out of school and just relied on large amounts of caffeine.

Boy was I wrong about that. I’ve had migraines since I was 18 and last year (after I had COVID, ofc) they became much more severe and are now accompanied by debilitating vertigo. Caffeine is a huge trigger for the migraines and vertigo so I completely cut it out. After cutting it out my ADHD symptoms really hit me like a brick wall and I found myself nonfunctional without caffeine. So a month ago my psychiatrist put me back on 10mg of adderall XR after being off it for eight years. 10mg works a bit but hasn’t worked as well as I’d like, so some days I double up and I found that 20mg helps me function optimally with no problems.

I got a new bottle on Monday and 10mg did nothing. I barely felt it. I even fell asleep. Today I took 20mg and felt it a little more but still fell asleep. What is going on? I definitely know I need 20mg for optimal function but 20mg isn’t even working. I also don’t take it on Saturdays so I don’t know how I could have built up a tolerance so fast when 20mg worked with no problems for me for years at a time. Does this have to do with the shortage? I’m so distressed I really need this to function.
