Because you aren't enjoying the things you're doing whilst avoiding more pressing matters. You sacrifice doing anything because you know you haven't earned it and you haven't finished your responsibilities yet - even though you know you're wasting the time anyway.
I think people assume you must be enjoying the procrastination and just don't care that much, but the experience is actually so stressful that just doing the task would be less so.
Only on medication does your mind seem to work with you. Even if you do get started later than you should have, your brain doesn't sabotage anything. You feel in total control.
