Chastity Cages

Does anybody have any advice for the current market for chastity cages? I've tried three or four different cages over the years. I always seem to run into a sizing problem. Either the cages are so lose, they just fall off. Or, they're so tight that I'm risking a trip to the ER keeping it on. I even had a custom 3D printed one made this last time and even that was too big! Any advice from any experienced members?
