Chapter creation questions: first chapter master and homeworld selection

So when a new chapter is approved to be created, is one of the new marines chosen to be chapter master right away? Or would an existing marine of the same primarch’s descendants be assigned to the new squad?
Plus, do they pick their own home world or are they assigned a recruiting world to start with?
