In terms of grim darkness, how does the Imperium of Man fare in comparison to other regimes in fiction ?

Is the IoM the undisputed champion in terms of fucked up regime or are there worse than them in other known fictional settings ?

The Combine is IMHO the absolute champion of evil governments in fiction and it's not even close. They purposefully do the things the Imperium does mainly through negligence.
