There is this evidence however what caught my eye in particular was this
It's clear at this point that there are two hidden characters in Afterbirth, one being Keeper. Now this next part is just logical thinking but if there is another character, they would have to be rather important. A random character that doesn't mean much wouldn't fit being extremely hidden. The Lost plays a major role in the story of Isaac. Keeper/Greed plays a major role. The only other person that could really be worthy of being a secret character would be The Hush, leading me to believe that The Hush is the only reasonable choice for this second secret character.
