So, I 'forgot' to file my income tax returns in April. What are the repercussions?

I've never had my taxes professionally prepared, each year I use an online service to do things myself (typically HRBlock). This year was no different, around the beginning of April I gathered together our materials and completed the forms online. I ended up with a state refund and a federal bill (~$5K) which is fairly consistent with what we end up with every year. I paid HRBlock the filing fee and was done with it.

Fast forward to last night...wife and I are preparing documents for a mortgage application and she asks me to forward last years returns. I search my computer but nothing's there. No big deal, let's log on to HRBlock and download the forms. Well, when I log in I see that while everything has been completed I never clicked on the 'file' button. I could have sworn I completed everything a couple of months ago, but clearly I didn't. I run through my email, I have a receipt for the filing fee, but no confirmation that they were actually filed. Clearly I fucked up.

I click the 'file' button last night and immediate get an email confirmation that my federal taxes have been filed. That's great, but it's two months late.

Additionally, one of the reasons I was so sure I had finished this in April is because I remember paying the bill. Sure enough, when I look through my credit card statement I see a charge of $5,225 (exactly the amount on my return) credited to "US TREAS TAX PMT".

My questions are: First, what are the consequences to having filed two months late? Late fees? Interest on the bill? And second, how can I be sure that the payment of $5,225 was (or will be) credited to my tax bill? I honestly can't remember how I submitted the payment, but without a tax return associated with it where exactly did that payment go?
