If a 13 year old boy forced sex with a 3 year old boy (oral or anal) would there be obvious physical signs that a normal caregiver would notice bathing or changing the kid?

Occurs to me that a 13 yr old is still going to have a smallish dick. A 3 yr old boy typically is about 12-17 kg (24-37 lbs). and is 36 to 41 inches tall. I was a chubby kid, so I was probably 33 lbs and 38 inches, plus or minus.

For comparison a 13 year old boy is 80 to 120 lbs and is 4'10 to 5' 4" tall

I'm trying to understand an abrupt behaviour change when I was 3.

Before the event, I was a normal toddler. After, I insisted on being fully dressed, was withdrawn, decreased appetite and got upset very easily. These are all consistent with CSA. Is it reasonable for a boy to be raped and caregivers not notice?
