40k in debt, should I consider consolidation/forgiveness?

24M, making $45k/year. I owe my father 14k for a car loan ($1500 or so) and $12500 for a debt consolidation I did through him. Its on his HELOC which he let me borrow money from to pay my cards back January of 23ā€™. I believe its 11%? Card Balances DoubleCash, $9950/$10000, 29.24% Diamond Preferred, $7785/$7800, 29.99% Amazon Prime $5581/$5600, 27.49%

On hand Cash/Savings/total ~$80.

Acorns IRA: $4,345 Company 401k: $6,919

Girlfriend of 5 years and I just broke up, about to find a new apartment so CoL is indetermined but I know landlords that will rent to me, likely regardless of my credit. I donate plasma for $105 a week, and Iā€™m about to start ubering again for hopefully 100-200 a week.

I totally understand this is all my fault and that my completely reckless and out of control spending is the real issue instead of my income, will be seeing therapists and psychiatrists shortly (thankfully paid for) just looking to see how to salvage my situation without any help from my fam.
