Anyone know what’s happened here?

One of my favourite orchids has started to bloom, however, one of the flowers has grown without its lip. Is this just a random genetic defect/glitch in the matrix :p… or is it likely to happen again? (I bought it last year, this is the second time it’s bloomed since I got it). Wondering if it’s not had enough nutrients or something? I’m very much a novice when it comes to orchids.
