Dungeon delving MMO/online coop rpg with extremely hard end game

I'm looking for something where the entire game is centered around one big mega dungeon where the end game is extremely hard and requires lots of preparation and strategy to complete. Qualities that I would love but aren't absolutely necessary are:
- Resource gathering
- Survival
- Crafting
- Loot
- Player economy
