What do you do when you feel a relapse coming? Help

I know the answer is just to white knuckle it but its hard. its SOO HARD. When i feel like a relapse coming its like i cant think of nothing but relapsing. Yesterday, i felt it coming, i fought with my mind and went for a swim and a long walk, and i thought i defeated the urge. I was wrong, as soon as i reached home I relapsed. This is so hard. Idk what to do
