Student loan company wants documents listing all disabilities.

Title. Applying for student loan forgiveness for 100% P&T. They are wanting some type of document listing all of my disabilities. I have no problems with this and I explained the letters we typically get, benefits summary etc.
Is there a formal document that would suffice that anyone is aware of? Thanks a
