GroupBy Gallery, User signed form to pdf & attach to all grouped records in sharepoint list

I am trying to build a work flow and I am on the last step. I have a list of material that I grouped by the funding code in a gallery. I need the user to be able to select or click the > on the grouped gallery to open a form that prepopulates data from the grouped rows (funding code, list of material and type of transaction) then sign it. I'd like the submit button to generate a pdf of this form with the list of material and attach it to each row of the group in the sharepoint list as an attachment. Is this possible and does anyone have any advice or videos?
