New saying at my retail pharmacy when we see a crazy script. Been keeping track of “crazy” errors that I catch. Caught 40 errors this week 34 were from NPs, 2 PA, 4 MD/DO.

Some of my favorites include:

1 pt got sent in Amoxil, Augmentin, and doxycycline for a “toe nail infection”

Lisinopril 40mg 2 BID prn headache on a paper script with just the pts first name and todays date

Ranolazine 1000mg BID for a pt on simvastatin haldol metformin digoxin

I called back on a jardiance script and had to fight my way with MA to get to the NP who didn’t know the pt was on jardiance bc she prescribed empagliflozin
