How do I politely tell my coworker to stop making so much noise?

I have a coworker who sits in the cubical diagonal of mine. For months now he clears his throat every 5 minutes or more, and he does so loudly, often with mucus-y coughing thrown in, too. It’s disgusting and disruptive. I get that people need to clear their throats, I do it too, but I do it quietly or even silently.

He’s also always talking, whether it’s making calls that most of us would handle with emails, or even trying to strike up conversation when everyone else has their earbuds in and is trying to work in peace.

How do I politely, perhaps even anonymously, tell him to shut up and mind his noise level when clearing his throat?
