Asking this as someone who sometimes contributes to information hubs about NPD and this strange alternate version of narcissism that seems to have very few differences from psychopathy, particular factor 1 traits such as lying, manipulation, and the obvious one - a grandiose sense of self-worth. I understand a lot of these issues come up due to semantics and overreliance on several broken systems but really all the labels are hard to follow and there's not much to differentiate NPD from ASPD and psychopathy as is.

Malignant narcissist just seems like a fancy way of saying psychopath, but without the deviant brain structure. Perhaps this bunch of people could appropriately be labelled sparkling sociopaths, Charlie Sheen and Lindsay Lohan come to mind.
