How to go about obtaining a medicinal card (19m)

So I've had some serious back, head, and knee problems for a little under 2 1/2 years. I've tried physical therapy, medicaly perscribed pain killers, the works and now it's just beginning to become to much. How do I go about getting a medical prescription, because the only thing honest to my non existent god, that has worked was cannabis.

Also my depression/anxiety has begun to rear it's face again any info to go about this is helpful thanks guys :).

Edit completely forgot about the state I'm in. I'm in New Jersey. Am I missing anything else?
