How to prevent smudging/smearing?

I try to use minimal makeup and mostly just wear bb cream and concealer under my eyes, around my nose and on scars. Sometimes I'll use a little bit of blush and eyeshadow if I'm feeling fancy. I don't like using powder so all my makeup is cream based and I use my fingers for application. The concealer does tend to smear and get on white shirts. I'm thinking of getting a setting spray to help with this. Should this be enough or do I need primer and a loose powder in addition to the setting spray? TIA.
