Heard a faint rattling chain sound, checked my rear wheel to see if nuts were loose and…that part is gone

(Second pic for reference of what is gone) I tightened my chain a couple weeks ago. This morning I heard the rattling sound again from a loose chain, checked and the thing is gone. I must’ve not tightened it well enough?

I used a torque wrench and followed the manual, but that obviously didn’t work out. Already ordered the part, but I’m worried it might happen again. I don’t think I should use locktight or any sort of permanent solution. Anyone else by chance have this happen? Or any tips of tightening it better?
