How do I smooth out my plaster walls when repainting?

I'm moving into an old New England house with plaster walls. Over the years they've accumulated a lot of bumps and ridges from being patched, and it's very obvious (more so in person). What is the strategy for smoothing these walls out again as I repaint them? Some of the advice I've read assumes there's drywall underneath.
