How do you not be negative? Does no one else feel an impossible weight just always present

I’m on medication for depression, I don’t think this is entirely related. I know I’m a selfish prick, but I don’t see anything to be positive about.
For reference I am a 30 year old man, who has never been more alone. I’ve had 5 strokes, I’m not supposed to drink anymore. Oh and having just come out this year most of my friends haven’t talked with me since.
