Advice needed!!!

I've had a chronic anal fissure now for 2-3 years but only had it properly diagnosed by a doctor a couple months ago as I was a dumb teenager and embarrassed about talking about on and off bleeding from my anus to my parents or a doctor. Bleeding usually happens when I have larger BM and/or if I'm constipated. It's at the 12 o'clock position and although it can be painful when passing a stool, I never have any pain or real itchiness outside of going to the toilet. However, as a 19 year old gay guy, I really want to sort this out.

I've been prescribed rectogesis to use twice a day and stool softeners (laxido) which I have used a fair amount in the past as I had issues with constipation when I was younger. I have been using these for a couple of months but there is still bleeding whenever I have a larger BM. I feel it may be related to my diet but I find it almost impossible to manage as a uni student with assignments an other commitments. I need advice on what I should do/focus on?

I've seen people on this subreddit talk about anal dilation and wonder if that would help with my ability to pass larger stools. I also know surgery is an option but the idea of incontinence from LIS or botox scares me. What's should I do? :(
